Community Liaison

The primary focus and goal of the community-based liaison is to generate referral flow and admissions from non-hospital accounts.

Create Referral Alliances: By working to build partnerships through service to the community we serve.

  • Use community profiling to identify true service areas and open doors
  • Develop educational / service events to create awareness and good will
  • Build referral practices/process with other senior providers in the service area
  • Creatively engage target market to “choose” Centers Health as their provider before a hospitalization

Home Based Growth in Admissions and Referrals:

  • Work in coordination with center staff to address all home-based referrals
  • F/U on d/c patients for 30 day window opportunity
  • Work with Hospital Liaisons to secure potential admissions from pats d/c’d home that may be in need

Lost Business Opportunities and Repositioning Plan:

  • Routinely conduct a competitive analysis of competitors in the community, keeping a pulse on changes and new products etc.
  • Identify opportunities to regain/ redirect lost market share from our competitors
  • Work with regional and Marketing group to create effective sales tools to position Centers Healthcare uniquely

Assist with Advertising/ Media Presence

  • Work with Regional and GCNY to build center messaging and Advertising Plans
  • Head up the “One Day” testimonial process
  • Help build written testimonials and Good Outcomes press by working with the centers

Event Planning

  • Partner with Regional to host events that showcase our centers i.e. open house, CEU, product launches, etc.
  • Represent centers at community events and functions

Support Regional team sales:

  • Cross train in multiple rolls to serve as hospital back-up and support
