MDS Assessor

Triboro Center is hiring an MDS Assessor to work in-person at its Skilled Nursing Facility in Bronx, NY.

Required: Registered Nurse (RN) License

MDS 3.0 Experience Preferred

Responsibilities:

• Coordinate the MDS assessment process, ensuring timely and accurate completion of assessments

• Review medical records for supporting documentation needs to ensure accurate MDS assessment

• Collaborate with Interdisciplinary Team to ensure appropriate determination of Medicare Skilled Level of Care or Managed Medicare

• Communicate effectively with the Interdisciplinary Team

• Collaborate with the Interdisciplinary Team to develop and implement individualized care plans based on MDS assessment.

• Participate in quality assurance and quality improvement initiatives to ensure continuous improvement patient care

• Educate Interdisciplinary Team in relation to MDS process

Requirements:

• MDS 3.0/Utilization Review/Case Management experience preferred

• Valid New York RN License

• Long Term Care Experience

• Must be highly organized, professional and motivated

• Should have solid computer skills with:

  • Working with electronic medical record systems

  • Basic working experience with Excel, Word, PDF, etc.

• Excellent communication skills

• Must be a team worker
