Job Description

Triboro Center is actively seeking a Patient Care Technician (PCT) to work 11AM to 7PM at our Skilled Nursing facility in The Bronx, NY

DUTIES AND RESPONSIBILITIES

Provides assistance to residents with activities of daily living.

Provides proper supplies/devices and equipment as available to assist residents with their daily routine.

Assists collecting data such as height, weight, vital signs, blood glucose, intake and output and records data collected in EMR

Makes resident care rounds on the unit every hour or as directed documenting residents’ whereabouts and condition/status as ordered. Reports variances to the RN/charge nurse.

Obtains specimens, labels and transports same to laboratory refrigerator.

Processes EKG treatments, including maintenance of equipment, preparation of patient, performing EKG and follow-up documentation and processing of EKG if warranted.

Completes intake and output by emptying collection bags/items. Documents same on I&O record on EMR.

Transport patients, supplies and equipment.

Reports repair needs of equipment or in resident’s environment to charge/Nurse Manager.

Provides unit and resident care areas with supplies and equipment.

Assists in admission, discharge, and transfer of residents.

Provides care delivery to residents including bathing, feeding, assisting with ambulation and transfers to stretchers, chairs, etc., turning and positioning of residents as assigned every 2 hours or as otherwise directed; etc.

Reports change in residents’ behavior, physical condition or resident issues to the RN/charge nurse immediately.

Escorts residents off the premises as directed.

Secures resident property and valuables and documents the same on the patient property form.

Provides 1:1 observation for resident safety as assigned and as per policy if applicable.

Documents as per standard on observation flow sheets.

Adheres to time and attendance policies, and dress code including ID Badge.

Demonstrates the ability to interact positively with resident visitors and co-workers.

Maintains resident confidentiality.

Communicates with other nursing personnel.

Adheres to Infection Control Policies: Practices personal hygiene; hand washing

Reports any possible infection control issues to the Nurse Manager/charge nurse

Attends and participates in nursing staff meeting.

Works collaboratively with other members of the healthcare team.

Performs other duties as required.

Position Requirements

High school diploma, GED or equivalent experience.

Medical Assistant Cert or Patient Care Technician Cert.

A minimum of six (6) months of practical experience.

BCLS/BLS preferred.
