RN Admissions Nurse
Bishop Rehabilitation & Nursing Center is looking for a RN Admissions Nurse to work in our facility in Syracuse, NY.
11AM-7PM
Duties & Responsibilities:
Complete admission assessments for new residents and hospital returns, including starting the initial plan of care.
Help support and guide the Nursing Department as directed by the Director of Nursing.
Review PRI information and work with the interdisciplinary team to help coordinate the admission process.
Review resident records and complete audits to make sure facility standards and policies are being followed.
Review and update resident care plans when there are changes in the resident’s condition.
Assist with discharge planning as needed.
Complete all required documentation and charting in a timely manner.
Complete MDS assessments, care plans, and other required paperwork as assigned.
Help oversee and direct resident care when needed.
Maintain resident privacy and follow all HIPAA and company confidentiality guidelines.
Requirements & Qualifications:
Must have an active and unrestricted NYS Registered Nurse (RN) license.
Previous experience working as an Admissions Nurse in a long-term care setting.
Strong clinical assessment and critical-thinking skills.
Able to make sound decisions and work independently.
Strong leadership and supervisory skills.
Comfortable using computers and Microsoft Office programs, including Word, Excel, and Outlook.
Long-term care experience is preferred.
Strong communication and people skills.
