UAS Nurse

Northern Riverview Assisted Living is seeking a Part-Time UAS Nurse to provide care for our residents in Haverstraw, NY. This position involves educating patients and their families about prevention and promoting healthy habits. The ideal candidate will be a highly skilled and responsible professional who can deliver exceptional nursing care with minimal supervision. Compassion and a calm demeanor are essential, along with a commitment to consistently following health and safety protocols. The primary goal is to enhance patient well-being by providing high-quality care.

We are offering a $5,000 sign-on bonus for this position!

Key Responsibilities:

• Conduct in-person comprehensive clinical assessments for current residents based on UAS guidelines, policies, and state regulations.
• Develop service plans and assess eligibility for continued program participation through UAS evaluations.
• Assess potential new members holistically (clinical, social, and environmental factors) to determine the necessary level of services for their safety and well-being within the community.
• Collect clinical data from various sources to address the patient’s conditions, following established guidelines, policies, and clinical judgment.
• Communicate effectively with patients, families, providers, and relevant parties to ensure the accuracy of the comprehensive assessments.
• Demonstrate a deep understanding of the needs of vulnerable populations with chronic or complex bio-psychosocial conditions.
• Complete all assessments in alignment with regulatory standards and internal policies.

Qualifications:

• Active Registered Nurse (RN) license required.
• Proficiency with UAS-NY preferred.
• Strong communication, organizational, and interpersonal skills.
• Ability to work autonomously and efficiently.
• Competency in computer usage and English language communication.
• Experience in MLTC/Managed Medicaid preferred.
